Download the licence application form (excel format) and sent it:

• by e-mail (one copy only ; remember to keep a copy yourself) or to the Sacem-SDRM delegation office in your region (two copies)

• or by mail to SACEM – SDRM - Demande d'Autorisation "Œuvre par Œuvre" (two copies)
